West Sacramento Event: May 20th, 2019

The Yolo Coalition to Honor Choices is hosting this opportunity to talk about your wishes during a serious illness or at end-of-life.  Our goal is to provide education and support to complete your written directives, as well as to speak with your family and your physician about your wishes.  We’ll watch the Frontline film Being Mortal, followed by facilitated discussion.

Based on the best-selling book by Atul Gawande, MD, Being Mortal explores the hopes of patients and families facing terminal illness, and their relationships with the physicians who treat them.  See the film and be part of a national conversation that brings medical professionals and community members together around the shared responsibility of discussing what matters most to patients and families facing difficult treatment decisions and how to have these conversations ahead of a medical crisis.

The Yolo Coalition to Honor Choices was formed in 2018 to support advance care planning in the communities of Yolo County.  This collaboration builds on the work many organizations have already done individually.  Members include the Alzheimer’s Association, Citizens Who Care, Davis Community Church, Del Oro Caregiver Resource Center, Woodland Healthcare, Listen-Ink, Sutter Health-Davis, Yolo Healthy Aging Alliance, and Yolo Hospice.  Learn more at www.YoloCHC.org
